• RSVP to the ethnic dinner on Saturday night
• Order tickets to the banquet on Sunday
• Indicate how many people in your party will be taking the optional bus transportation between Holy Trinity and the Carlisle in Lombard.

Please submit your completed form with payment as per the instructions included on the form. We are offering you the convenience of paying via Paypal, however, please note that a transaction fee has been included. 

Use the drop down menu below to select and add the proper ticket option to your cart, and be sure to mail/email your completed form and itemized email receipt from Paypal to Sue Miller at the address included on the form.

We use Paypal to process payments using Paypal accounts or credit cards. 

1. Select the desired ticket option from the drop down menu below and click on the "Add to Cart" button to order via Paypal.  Paypal will then open a separate window in your browser so you can review your cart.  Be sure to send in your completed order form!  Please note a transaction fee charged by Paypal has been included.
2. You may purchase multiple tickets in one transaction.  Confirm the information is correct, then click the "Continue shopping" link in the upper right corner and you will be returned to this page to select another ticket option.
3. When you are finished adding banquet tickets to your cart, you can follow directions to check out with a credit card or a Paypal account.
4. Please email/mail a copy of your completed order form and the itemized receipt from Paypal showing proof of payment to Sue Miller.
